Terrible – don’t send your loved ones here! Management was a joke. Physical therapy lies about progress Front desk does nothing- just blank response or they just really don’t care Some nurses care – you can tell which care Poor communication Long call times Forgot to give medication once Bathrooms NOT up to code for guests A random patient spit on my car after putting their cigarette out on it 😡 Food sucks – I usually brought stuff daily Avoid! My car should have NEVER been used as an ashtray because my mom should have been discharged HOURS prior I’ve called and emailed multiple times just to be ignored… AVOID this UNPROFESSIONAL JOKE!!! 😡

I recently had a hospital stay and, when it came time for rehab, the hospital recommended Vasona Creek Health Center. I looked at a few different options in the area, but Vasona Creek seemed like the best fit. Looking back, I’m glad I chose them. The transition from the hospital to Vasona Creek was smooth and well coordinated. It was clear that the hospital team and Vasona Creek communicated well with each other, which made the process much less stressful…. What impressed me most was the quality of care and the overall environment. The rooms and common areas are clean, comfortable, and welcoming. The rehab department is excellent, and the therapists genuinely cared about helping me regain my strength and independence. What really makes Vasona Creek stand out, though, is the people. Every day I was treated with kindness, respect, and patience. I would especially like to recognize Diane, Henry, and Hue for always going above and beyond. Nurse Christine was wonderful and provided exceptional care, and I definitely can’t forget Rudy, who always brought a positive attitude and made my days brighter. I also want to thank Mark and Omar from administration. It meant a lot that they took the time to check in on me regularly and make sure everything was going well. Those small gestures made me feel valued as a person, not just another patient. Recovering after a hospital stay isn’t easy, but the team at Vasona Creek made the experience as comfortable and positive as possible. If you or a loved one need skilled nursing or rehabilitation services, I would absolutely recommend Vasona Creek Health Center!

Despite being to many other centers, vasona stood out the most to us and my father. My father was very happy with his stay and the staff and nurses showed so much care for him.

My husband is currently a patient there. One of his roommates has been harassing him. He came out in one of my husband’s shirts, when confronted he took it off and threw it at him. The next night my husband woke up to find him going through the items on his bed table. Last night e put underwear in my husband’s bed. I have complained and nothing has been done. It is not safe.
I would never recommend Vasona Creek Healthcare Center to anyone I care about. Based on my experience, I was deeply concerned by the facility’s priorities. It often seemed that discharge decisions were driven more by insurance coverage and reimbursement than by whether residents were actually prepared to leave. In my experience, patients without Medicare coverage appeared to be discharged very quickly, and many Kaiser patients also seemed to have very short stays, regardless of whether they or their families felt they were ready. The leadership, in my opinion, lacked professionalism and accountability. I felt that important decisions were often influenced by individuals outside of their appropriate roles rather than by what was in the best interest of residents. When concerns were raised, I rarely saw ownership or meaningful action. Instead, blame was often shifted, and problems continued. While there were a handful of employees who genuinely cared, I felt that too many staff members had become disengaged. Residents deserve compassionate caregivers who advocate for them, not a culture where concerns are overlooked. Without fail, Vasona Creek administration and management will prioritize profits over residents’ dignity and basic needs— even when the law requires meeting those needs. I also witnessed repeated issues involving delayed maintenance, missing belongings, communication failures, staffing concerns, and residents’ safety, YES, RESIDENTS’ SAFETY! I cannot express that more clearly. CDPH and Ombudsman, if you call them, they will come and take a look and speak to people, but the concerns never get resolved—at least the not in the way you hope. Vasona has a way of covering up very serious issues and administration and management weasel their way through investigations. All employees know when CDPH and the Ombudsman will enter the building — everyone scrambles to put a great act on. These were not isolated incidents, but part of what I experienced during my time there. I left feeling that the residents deserved much better. If someone I loved needed skilled nursing care, this would not be the facility I would choose.

Worst communication ever at this facility. I have been in health care for over 20 years and have never had a worse experience with my family member’s care. Took me about 2 weeks to get a hold of Dr. Toram. His number says to leave a message or text him. He ignores most of my messages and has hung up on me twice. Very unprofessional! He is sarcastic, down plays my concerns, and does not seem to want to do his job. The other issue is nursing staff. My family and I have left messages for nurses, told we would get a call back, and in almost 2 months, not one nurse has called us back. The only call I got from a nurse is when my grandmother fell. I have left message for director of nursing Sheila, and no call back. When you are transferred to the stations no one ever answers, although when I go in, there is almost always at least 1 person sitting there. I think the problem is there is no accountability for anyone at this facility. If they don’t follow thru with what they say they are going to do, act professionally, or provide the care they are required to, no one does anything about it. The only 2 people I can say that has been somewhat helpful and returns my calls or messages are Donna in social services and PT director. Also, Noha in PT and OT staff are really good. I understand that most of these facilities are busy/understaffed, and I have been trying to be very patient, but this care has been unacceptable! Most likely this review will just get ignored by management. However I wanted to post for others that are experiencing the something here or thinking of brining their loved one here.

I am currently a patient here and good and bad 1. Food is bad most of time you can’t tell what it is 2. Nurses are scares. A few goods one on top of everything especially with meds then they bring in contracted nurses who take forever not sure why they don’t have all the meds already sorted and a 2nd nurse would be of great help 3 they are never on time with meals 4. PT and OT are great I actually enjoyed my times with them At this time I have no weight bearing but I so want out out of here asap At least so I can get my meds on time Not the best not the worse Claudia and Ms Joy great people
I had a disappointing experience here, largely because of the management. There were repeated issues with communication and organization, and when concerns were raised, they were either ignored or handled unprofessionally. Staff seemed unsupported and unable to resolve problems effectively. As a customer, this resulted in unnecessary delays and frustration. Based on my experience, management needs significant improvement in responsiveness, accountability, and customer service. Nurse: Shiela and Sean are super bad!!!

VERY BAD FACILITY!!! DO NOT BRING YOUR LOVED ONES HERE! The Director of Nursing Sheila does not do anything. She does not respond to any of my texts or calls and when I stopped by in her office, she was so relaxed with her legs even up in office chair. The facility smells strong urine every single day. My dad’s curtain was even stained with feces. CNA says that they are short staffed and don’t even have time to help out with my concerns. Nurses does not follow up with my dad’s medication on time and was always missing. Group of nurses are always in one station having a good time when my dad’s call light’s on

I will update you. They put a response about caring however I called multiple times to try to resolve my concerns so future families do not have these issues. No answers and no return phone calls. The upper staff do not care.

Been here over a week as a patient, and im so weak that I wish I were dead. Not enough staff. Medications are late or inconsistent. You have never seen so many tattoos in your life. You can ring buzzer for help and wait 19 min to 60 min for response. My blood sugars are put of control. Some pts and aides were helpful but because the patient to caregiver ratio was so inconsistent, I just said I wanted death rather than to continue here. Last night my Ethiopian livn spilled juice all over my tray and didn’t clean it up or bring me juice. Food was so awful that I ste the t Roll n asked for yogurt n banana. As a current patient i can express myself unlike those recovering from a stroke. Terrible. I WISH THE STAFF COULD SPEND A DAY OR WEEK HERE. THEYD BE SCREAMING THEIR HEADS OFF. Website shows a pool. No pool.out of service for almost a year. No plans to fix E3b S How difficult it to locates my missing laundry . Nightgown, tshirt My stomach is cramping because they want to wait after breakfast to change me. They forgot my eve rx and morning rx. Blood sugar was 255. Woopie probably be igno U tell me to call but ur internal wifi is so bad it won’t go through. Sunday I had poop in my diaper twice. Lvn acted like it was my fault. Why didn’t she get me a nurse??? Instead I. Will probably be ignored until next bout of diahrea. I feel like an an animal but animal get treated better. This morning I tried to use the commode. I could only pee not poop. After pt I asked to use commode again but they had not cleaned it. Made me ill. At this point I’m going to a cheaper place of my sisters choice. I’ll be dead soon.

My Father is a current patient at Vasona Creek and I’m making a heartfelt review about the admissions director Omar Rumero. I heard how he talks to other patients and he is very kind and passionate about his job. I appreciate all the good services and discussions that we’ve had, he has done an excellent job! I also want to highlight how full of life the facility is with engaging activities and a top quality rehab team. I have visited a nearby skilled nursing facility and I wasn’t impressed – Very disappointing with ridiculous fees for a private room. Choose Vasona for all your skilled nursing needs. A plus: Omar and the team worked very well with the hospital and the whole process was just wonderful!

This place is horrible at answering the phone at all 5 nurse stations. I’ve called and left multiple voicemails and I still haven’t received any call and it’s been over a week. mind you I left voicemails to 3 different people including the supervisor, not one person has called me back! They don’t communicate with their patients at all and they also give them medication that’s not on their chart. When I called to see what medication they gave my father they only had 1 in his chart. But they gave him more than 1 pill? This place sucks!!!

Staff were top-notch. Caring, knowledgeable, supportive, professional and did whatever it took to encourage the patient to do his or her very best. Kudos to them!
Once you get to know the lvn’s and nurses they treat you really good and the physical therapy and occupation therapy are great and the memory care is amazing for only having one person who does it for all the patients that need it the place is always clean and I appreciate my stay to recover after surgery and I almost didn’t make it out of surgery so I needed to learn everything again last but not least the activity team is amazing from bingo to chair yoga and arts and crafts plus games I would like to say thank you Vasona creek Healthcare

I’ve had the best experience here!!! took my grandpa here and the staff were incredibly nice, we felt very welcomed here!!! I know 100% my grandpa will be under great care!!!

DO NOT BRING YOUR FAMILY MEMBER TO THIS AWFUL PLACE!!!!! When you want to transfer your family member from this hell hole the social service team will lie to you about helping you transfer. When requesting your family’s clinicals Kyla in social services will only give you half of what you requested. Kyla and her team will do everything they can to make you stay at this HELL HOLE! This place is loud and smell like feces and urine, very noisy. out of control patient and aggressive that will try to confront you! When you need to have a changing for your family member it will take between 20-30min urine/feces to get changed and wound clinic will not check the wounds as scheduled. When you want a nurse no nurses are to be found!

Do not send people you care about and love here. If you complain about bad food (spoiled), they dismiss it and say well if this person was home then you can control the nutrition they receive. There is one wing that you can tell is neglected and smells of rancid pee. The rest of the wings you can tell the staff cares. This is not put down the great employees that we encountered however there is a nurse and few others that dreads changing her patients when they have diarrhea that the food they served caused. Also, on the first floor at least 5 employees just sitting around on their phones. If they are on break there should be a break room area for them. Multi people were screaming for help on numerous occasions. Health inspector should investigate.
